PSC provides certified geriatric care management services through:


* Respect for client autonomy


* Consultation and initial assessment to identify strengths, areas that need support, and recommendations / suggestions.


* Listening to each client and their family


* Assistance in enhancing quality of life


* Generating plans of action to realistic support and meet their goals and desires.


* Assisting families dealing with challenging dementia issues.


* Coordinating collaboration with medical providers


* Optional on-going supportive counseling and/or case management oversight

WHO WE ARE


Anne has over 35 years experience in administrative and clinical positions in acute care and nursing home settings, as well as 12 years as a dedicated geriatric care manager. She is a licensed Clinical Social Worker in NY State, a Board Certified Diplomate with the American Board of Examiners, and holds certification as a Certified Geriatric Care Manager and Advanced Clinical Social Worker through the National Association of Social Workers.

Anne is a member of the International OCD Foundation and is considered a 'Hoarding Specialist" and is also the Executive Director of the Hoarding Disorder Resource and Training Group.

In addition, for 9 years Anne taught integrative seminars in gerontology at the Fordham University Graduate School of Social Services, and has been a member of the Fordham University Ravazzin Center on Aging. She has assisted in teaching the Article 81 Guardianship Certification course for attorneys and families at the Brookdale Center for Healthy Aging and Longevity.  She is also specializes in dementia care and services.

Anne holds a Master's degree in Clinical Social Work from Simmons College in Boston, MA, as well as a post-Master's Certificate in Aging from the Brookdale /Hunter College Center on Aging in New York, NY. An active member of the National Association of Professional Geriatric Care Managers, Anne has served on the editorial board of the Inside GCM publication, and on the Educational Alliance Committee.

Aging Life Care Experts are professionals who can assess older adults and develop a care plan that will determine the need for in-home care, relocation and additional medical care. Aging Life Care Experts coordinate a variety of care related services and can be the eyes and ears for family members who live far away. They are also there for those family members who live nearby, but are too busy to handle the daily details of the older person’s life. 


Aging Life Care Experts provide the answers and the solutions to busy family members who know help is needed but don’t know what to do. Aging Life Care™, also known as geriatric care management, is a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals provides the answers at a time of uncertainty.



An Aging Life Care™ Expert is a professional who specializes in assisting older people and their families in assessing, coordinating and planning care. Aging Life Care™ Experts have training in gerontology, social work, nursing, and/or counseling.


Aging Life Care Experts can help:


  • Conduct care-planning assessments to identify problems, eligibility for assistance, and need for services
  • Screen, arrange and monitor in-home help or other services
  • Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
  • Provide crisis intervention
  • Act as a liaison to families at a distance, alerting families early to problems to ensure early interventions
  • Assist with moving an older person to or from a retirement complex, care home, or nursing home
  • Provide consumer education and advocacy
  • Offer counseling and support
  • Some Aging Life Care Experts can also provide family or individual therapy, money management, and conservatorship or guardianship assistance.
